Transition education is recognized as a necessary educational and therapeutic process to support the ongoing health of teens, beginning around 15 years old. Because your child has a CHD it is especially important that they learn about their health and how to manage it themselves.
Congenital heart disease (CHD) requires lifelong follow-up to prevent complications such as arrhythmias, heart failure, infection, stroke and premature death. The goal is to move them from a dependent role in their healthcare, to one of independence and self-care, according to their level of ability.
